ITEMS NEEDED TO ENROLL AT NORTHSIDE MIDDLE SCHOOL

Below are the items you’ll need to have to register your child at Northside
Middle School. The report card and standardized test results are necessary
for class placement.

In general, the process should take approximately one hour; less if all forms
are completed:

1.Completed registration packet forms

2.Two Proofs of Residence: One Utility Bill and a Sales Contract on a
House or an Apartment Lease Agreement

3.Child’s Birth Certificate

4.Child’s Social Security Number (if available)

5.SC Certificate of Immunization (within 30 days)

6.End of Year Report Card

7.PACT Test Scores (if from within South Carolina) or other nationally
standardized test scores if student is from out of state.

*Parent or legal guardian MUST be present for registration; the student
may come as well.
